Hello,I want to thank everyone for taking care and befriended my husband during his stay at the rehab. My husband spent ten months at Nassau Rehabilitation Center and giving five stars is not enough.
On the first overwhelming day at the rehab I met a nurse, Ms. Harvey. As I turned to leave my husband’s room, she opened her arms and I received a much needed bear hug and I knew he would be okay at Nassau.
I went on to meet several caring people. But none compare to Mrs. Cheryl Weathers, the financial coordinator extraordinaire. She is caring, reassuring, patient, and helpful beyond words. I will forever be grateful. She’s become my go-to peroson when his social worker- Charlene was not available. Cheryl gives herself unselfishly and has made herself available even on her day off. She is truly an asset to the rehab.
I can't thank enough the awesome nurses-Ms. Weslyne and Ms. Melissa who used a bit of gentle coaxing so my husband can take his meds. Nurse Harvey, and mom Harvey who would give me comfort when I needed it most and talk to him about food. His physical therapist, the great Ms. Carmen, who would literally lift him and encourage him to walk. As for the CNA 's: Ms. Antoine- an angel from heaven who took great care of him from day one and went over and above to assist him, Ms. George & Ms. Thomas who knew how to calm him down when other CNA’s could not. Thank you for your patience and tender care that you gave him. To director of Rehab and your super team...THANK YOU.
And lastly a suggestion on how to improve discharge. Having gone through the process, may I suggest the social worker be more proactive with the family once the discharge date is agreed upon.
The insurance company should be called to ascertain what equipments are covered. The social worker should be fully aware of what equipment one’s insurance will cover & inform the family with enough time to decide if they are willing to pay or not. Since the social worker is (should be) aware of patient- state of mind, movement difficulties, stiffness, rigidity- some equipment should be offered. If payments are involved, spouse should be informed in a timely fashion. Social worker should in constant contact with equipment company to get an update on status of delivery. Once the social worker receives the notice that all equipment are available, delivery should be delivered to the patient’s house at least 2 days before discharge. At no time should a patient who MUST have a hospital bed be sent home and the bed is not available.
I hope these practical suggestions will help to improve the discharge process.
Regards,
Mrs. Louis

My aunt was a resident at Nassau Rehabilitation Center. She was admitted for short term rehab. The facility was clean and the staff were helpful. I dropped in on their Halloween event. So cute and fun for the residents. I would recommend this facility if anyone asked.
My husband has been at Nassau Rehab for over a year and we love it here. He get’s the best care from the staff and it’s clean. I’m here every day and the staff help me get through my ups and downs.
