Park Avenue Extended Care

Most of the staff members I've encountered have been wonderful and caring. The PT is excellent and the food is quite good with a nice variety. I'm a success story of Park Avenue Extended Care Rehabilitation Center. After a skin graft to repair a wound on my foot I entered the Center on 10/21. It wasn't until 12/21 that I received clearance to put weight on that foot. Fortunately the wonderful therapy staff had been giving me exercises to prepare me. I've now gone from standing to walking with a walker unescorted. The care I've received from this facility has been excellent. The nurses and Aides are friendly and compassionate, the physical therapists are top notch and the food is quite good.
If you are in need of this type of facility place Park Ave. on the top of your list. They are located at
425 National Blvd, Long Beach, NY 11561 rehab or hospital is 100 percent but this facility ranks very close.

My mother, after being discharged from the hospital, was sent to Park Ave Rehabilitation, to continue treatment and regain mobility. From the moment she arrived, she was greeted by caring and professional members of the staff. From the concierge, to to the nurses, doctors, aides, social workers, physical/occupational therapists and everyone in between, my mother was treated like a member of their own family. The facility was kept extremely clean, and the food was good as well. The environment was comfortable, and conducive toward healing. After six weeks of rehabilitation, my mother was discharged, and on the last two days, I watched individual members of the staff, all come to say their farewells, each time, evoking tears from my mother, which proved to me, just how wonderful the staff truly is. Thank You Staff for bringing my mother back to health. You are all special to me, and I’ll always be grateful.

After having had one of the most horrific experiences at the hospital and the previous rehab facility, our father was able to finally be at peace here at Park Avenue Extended Care. Although he was only there for a short amount of time, the level of professionalism, knowledge, and compassion proved to be second to none. We so appreciate the staff at this facility for all that they did to keep our father out of pain and to finally be at peace. I have been here over 6 weeks recovering from a hospital stay. The staff has been very caring and the care diligent. My only complai ts are that their staffing levels should be higher. The nurses are overworked leading to long waits occasionally. Otherwise a good facility. I highly reccommend it.
