PARK PLACE HEALTH & WELLNESS CENTER located in St John, IN is located on Rt. 231 just East off Rt. 41. I have had the opportunity to visit the rehabilitation center as my Mom is currently recommended. My first impression is how courteous the staff is. Each is thoroughly respectful of your loved ones & their individual needs. A meeting follows your admission with the social networking staff & physical therapy department. Information on your stay & protocol is discussed & any questions are addressed. The center itself is clean, cheerful & feels more home inspired rather than a healthcare facility. My Mom is my main priority & she is happy at Park Place Wellness Center. She comments frequently on the homelike atmosphere, caring staff & good food. I joined a group exercise class head by 3 therapists using Therabands to promote strength & flexibility the other day. This was a good workout! I wanted to thank Dee (social networking staff) for taking time out to explain in detail the avenues of care available following physical therapy to me. Her friendly & caring demeanor helped ease my mind regarding future care. The protocol for your loved ones safety is primary & I will share admittance into the center for visitors. Masks are required to be worn .. you will be required to sign in, record who you are visiting (rm #) with a time in & time out. Separate forms dated & signed for each individual visitor stating current health & temperature recorded by staff before admittance. They will only then activate the doors or elevator for you. I feel this is but a small sacrifice to ensure the utmost safety of the residents. I review businesses & other facilities on Google that I have visited & am not affiliated with any.

Very happy with the communication and care. I am located in another state, and two separate staff members, Dee and Dave, called me with information about a loved one from their personal phones after normal hours. This was very helpful during a challenging time.

My husband received excellent care at Faith House. They were sensitive to his needs and ours in his last days.

My Dad was a short-term stay resident in the rehab facility after a hospital stay. As soon as he arrived there, I was received an email and a phone call to discuss his needs. The email had phone numbers for the front desk, the nurses station, and his social worker. The nurse gave me her cell phone number right away and was texting with me through the first day and evening to be sure everything was set up and to reassure a worried daughter. Everyone I came into contact with at Park Place was kind, helpful and caring. The daily COVID email updates as well as follow up phone calls regarding any cases were very appreciated. All safety protocols were followed and I felt confident that Park Place was doing their best to keep all of the residents safe and healthy. Dad was well taken care of and he was always very happy on the phone and during an outside visit. His therapy went very well and I was informed about his progress weekly. Everyone, including the director, nurse and social worker were very prompt to reply to any email as well as any voicemail. Upon his discharge, I was given detailed discharge information as well as all necessary medications and prescriptions. Park Place also calls the caregiver for a few days after discharge to check on his progress at his new living arrangement. I highly recommend Park Place.
