Chisolm Trail Nursing & Rehab Center

I am a hospice nurse so I go in and out of Assisted Living facilities, memory care facilities, skilled nursing facilities and nursing homes all day long. There are very few that I would even consider putting my mother in, but this is one of them! I placed my mom in Chisolm Trail Nursing and Rehab and we had the best experience possible. The staff are friendly, knowledgeable, loving, personable and engaged with their residents. I highly recommend Chislom Trail to anyone that wants to place their loved one in a clean, loving environment!

My past experiences with nursing facilities had not been pleasant to say the least. We had just moved to Lockhart TX, found this jewel of a nursing facility for my sister. I live 2 minutes away, I’m in the facility several times/hours during the week. I can honestly say, the staff have been consistent, not a high turnover. The whole team at Chisholm Trail take very good care of my sister. When I walk in the facility, it’s presentable and clean. They treat visitors with the upmost respect. If any concerns arise they are promptly addressed. The professionalism of the Staff at Chisholm Trail is of the upmost and I am so very pleased with all of their efforts. It surpasses all of my expectations and I have absolutely no reservation in recommending Chisholm Trail to anyone in need for the care of their loved ones.

AMAZING HOME for my precious family member. When you have to make the decision for long term care for a loved one it’s very hard. Our loved one is deaf & mute and so the decision was 100 times harder. From the first visit the team at Chisolm Trail made us feel very welcomed. They made us feel like family!! Many of the team at Chisolm have been employed there for 10+ years and you can tell they love what they do and they care & love the residents. Going above and beyond with communication charts, her likes and dislikes, therapy, activities, and so much more! Every time I visit my loved one, she is beaming, the team welcomes me and I can see that she is very very happy! The business office mgr, social worker and administrator have been a tremendous help in answering a million questions for me. The nurses, rehabilitation team, aides and facility staff have been extremely helpful and so loving to my aunt. The team takes pride in the facility and it is important to them to foster a sense of family and home, and it shows by how they maintain a very clean space and decorate for every occasion. I could not imagine a better fit and I’m so thankful for the entire team at Chisolm for helping our family transition in such a positive way. I highly recommend!!! ⭐️⭐️⭐️⭐️⭐️

My mother has been there for almost 3 years and I couldn't be happier. The staff is friendly and supportive and the Center is really clean. My mother couldn't be happier. I am so grateful she has made Chisolm Trail her home!

The management and staff at Chisholm Trail Nursing and Rehab Center are very friendly and helpful. You can tell they really care about the residents and the safety within the facility. Having that peace of mind knowing your loved ones are being taken care of is how every nursing care facility should be. I would definitely recommend family and friends to this nursing care center!
