A family to me. The only reason I would leave Autumn Leaves is if I could be in the city my daughter resides in.
However it is in another State and I

(June 14, 2018, 5:27 pm)Employed at Autumn Leaves since October of 2017. Having been in long term care for 15 years I can say the DON and nursing staff are very compassionate and go well above the norm to ensure the residents are cared for. The leadership team work as a cohesive unit to ensure the customers needs are meet to the best of their ability. I have received numerous positive letters, cards etc. from current and past residents/ families regarding the care and expressing their gratitude for the care their loved one received. So much so that we created a display album in our lobby to share with others. In addition, we have a number of previous residents who return when they require nursing care, rehabilitation etc. Very proud of the team here!
(May 5, 2018, 3:40 pm)I have visited this facility several times and on each visit Autumn Leaves has been very very clean, the staff is amazing and very friendly. On my visits I also noticed that the residents are very friendly and seem to be very happy at their home. There are always activities and events posted for the residence they also schedule trips to the grocery store. I think Autumn Leaves is a wonderful place for the staff and the residence.
(April 24, 2018, 7:37 pm)My Grandmother had a successful stay at Autumn Leaves after a stroke. The rehab team and nursing did a wonderful job with my Grandmother. They often have entertainers for their residents, to make their stay a little more easier. I highly recommend. Beautiful view of the White Rock Lake.
